CHATSWORTH ROAD MEDICAL CENTRE - PATIENT PARTICIPATION GROUP

The Patient Participation group works with the practice to find ways to improve and maintain the high standards of care patients have been used to.
We meet Doctor’s and practice support staff on a regular basis when ideas and any service issues are discussed. It is important for the group to understand methods and practices that work and, at times doesn’t work.
This short survey is designed to help us understand how patients view methods adopted by the medical centre staff as a result of the Covid-19 pandemic.
This survey is anonymous and can not be used to raise individual complaints. If you have a complaint, it should be submitted through the complaints procedure at the medical centre.

When you first make contact with the Medical Centre would you be comfortable being asked about your medical condition by a receptionist? This would assist in ensuring contact with the appropriate medical professional. (Contact could be by telephone or in person.) If you would like to elaborate on you answer, please do so in the box below.

Would you be in favour of being seen/assessed by a nurse before being referred on to a doctor if necessary? If you would like to elaborate on your answer, please do so in the box below.

Would you feel confident in explaining your health condition to a medical professional on the telephone? If you would like to elaborate on your answer, please do so in the box below.

Do you consider it is necessary to be seen by the same doctor on every occasion?
