Patient Survey Autumn 2013
Last year our patient survey focussed on the way we communicate with you in person and on the phone. This year we are focussing on your experience of seeing our doctors and nurses. We want to provide the highest standard of care. Feedback from this survey will help us to identify areas that may need improvement. Your opinions are very valuable.
Please answer ALL the questions that apply to you. There are no right or wrong answers and your doctor will NOT be able to identify your individual answers.
Please tick in one box for each question, except where mentioned.
We will keep your answers completely confidential.
